The challenge
The clinic serves patients who search with Rio modifiers and service terms. National wellness blogs with no geographic relevance were not moving those results. Prior outreach had chased high-DR national health sites that accepted almost any contributed post. The links looked strong in a tool and weak next to the queries that book appointments.
Medical claims required careful wording. Marketing wanted speed. Clinical review wanted restraint. The campaign had to satisfy both, in Brazilian Portuguese, without sounding like a translated US wellness article.
The inherited file mixed directory leftovers, thin health blogs, and a handful of solid mentions that had nothing to do with Rio. Leadership wanted a publisher file they could defend city by city, not as one blended health chart.
The approach
Geography led the brief. Prospects prioritized Rio health sections, Southeast Brazil lifestyle titles, and business journals covering healthcare employers in the metro. Generic national wellness mills were filtered out before the client saw a sample.
Criteria covered niche, Ahrefs DR, Moz DA, Majestic TF, and a traffic floor. A sample list with full metrics and pricing went out usually within 24 hours. The clinic built an approved pool of roughly twice the monthly target. Destination URLs were the location hub and a small set of service explainers cleared by clinical review. Anchors stayed branded or geographic; exact-match medical keyword spam was refused.
Every host was hand-vetted: archive quality, outbound neighbourhood, and whether bylines existed outside the CMS. Thin sites and PBN patterns were removed. In-house Brazilian Portuguese writers drafted articles past 1,500 words unless a shorter length was requested. Anything that sounded like a treatment claim without clearance was rewritten or dropped. Live URLs were logged in a shared Google Sheet as they published.
Results
Over 7 months, organic sessions on the location hub and cleared service pages rose 46 percent. Appointment-related landing pages saw a matching lift in non-brand clicks from Brazilian Search Console for Rio-intent queries.
Regional editorial placements on the approved pool reached 22. National wellness residue stopped being added. Referral traffic from accepted hosts was small and high-intent.
Clinic marketers finally had a publisher file they could open in regional leadership meetings: city by city, with every live URL visible in the sheet, not buried in a PDF.
What we declined
- National wellness mills with no Rio or Southeast Brazil relevance.
- Thin health blogs and directory-style hosts that failed the hand-vet.
- Exact-match medical keyword anchors and unverified treatment claims.
- Spanish Latin American health content pointed at Brazilian patients.
Closing notes
Clinical review remains in the loop. Speed never overrides clearance. That is why the pool is sized at about twice the monthly target: desks and reviewers both get room to say no.
The engagement continues with the same geographic filter. New Rio-relevant prospects enter the Google Sheet each month so the file does not freeze around the first sample list.
